You are hereby consenting to allow chrisad, Inc. to use and disclose your testimonial, audio, photos and/or videos and you acknowledge that they may be distributed to the public.

Right to Revoke: You have the right to revoke this Release at any time by providing written notice of your revocation and submitting it to the Contact Person listed below. Please understand that revocation of this release will not affect any action chrisad, Inc. took in reliance on this release before receiving your revocation.

CONSENT TO RELEASE I hereby authorize chrisad, Inc. and staff to use my testimonial, photos, videos, audio and any information contained herein in its media/public relations efforts. I understand and approve the disclosure of the testimonial, photo, video, audio information to the media and other individuals and entities that may be involved in the media/public relations efforts of chrisad, Inc..

I understand my testimonial as outlined above (the “Testimonial”) and made on behalf of chrisad, Inc. (hereinafter called “chrisad, Inc.”) may be used in connection with publicizing and promoting chrisad, Inc.. I authorize chrisad, Inc. to use my name, brief biographical information, and the Testimonial as defined on this form.

I hereby irrevocably authorize chrisad, Inc. to copy, exhibit, publish or distribute the Testimonial for purposes of publicizing chrisad, Inc.’s services or for any other lawful purpose. These statements may be used in printed publications, multimedia presentations, on websites or in any other distribution media. I agree that I will make no monetary or other claim against chrisad, Inc. for the use of the statement.

In addition, I waive any right to inspect or approve the finished product, including written copy, wherein my testimonial appears.

I hereby hold harmless and release chrisad, Inc. from all claims, demands and causes of action which I, my heirs, Practice Growth Analysts, executors, administrators or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
